The Five Practice Changes
Using the concept of implementation science, the CHCA has identified five overarching practice changes and the steps necessary to develop and implement the evidence-based module of Whole Community Palliative Rounds.
1. Enhance the Circle of Care
Define community partners and review privacy legislation.
2. Identify a WCPR Facilitator
Select a facilitator and detail their role in managing the rounds communication process.
3. Get referrals to WCPR
Encourage clinicians to refer appropriate patients to the rounds.
4. Engage Partners in WCPR
Ensure appropriate clinicians attend and participate in rounds by sharing value statements and benefits for patients.
5. Actionable Recommendations from the WCPR
Promote communication and actionable recommendations from the rounds.
