Defining the Enhanced Circle of Care
The first practice change in implementing Whole Community Palliative Rounds (WCPR) involves identifying the health and social care providers who should be part of a patient’s circle of care and understanding the rules for sharing health information within that circle of care.
A “circle of care” is a group of health care providers supporting or treating a patient who might share information in order to provide that care. WCPR uses a broader circle of care than other case conferences to encourage interdisciplinary communication, collaboration and shared decision-making.
